When possible, the submitted information is published in the ROOT CELLAR PRESERVES. The data you furnish will be a valuable addition to the history of California and is a source of information to members of your family for years to come.

You may submit as many applications as you wish, but remember that only one name on each form.

(Use a separate form for each surname.)
  1. Include a Pedigree Chart(s) four or five-generations Use as many sheets as necessary.
  2. Include a Family Group Sheet for each family to show your direct descent.
  3. Include any relevant photographs if available. Label with names, dates and locations.
  4. Put your name and address on EVERY PAGE THAT YOU SUBMIT.
